Forms and Records Submission

Please refer to the letter you received regarding your audit and follow the appropriate instructions below to submit your audit records.

If you’ve lost your letter or are uncertain how to submit your audit records, please call your Premium Audit representative or call (888) 782-8338.


Use this option if your letter instructed you to download the fillable audit forms (found at the bottom of this page) and upload them with the records requested to the Secure File Upload Portal.


Use this option if your letter instructed you to login to the Audit Portal. Your log in ID and temporary password are included in the letter. If you do not have your letter or ID and password, please call (888) 782-8338.


If you plan to mail your audit records, download the audit forms below and mail them with the records requested to:

IMPORTANT: When mailing documents, please send copies only, not originals. Records will not be returned; they will be scanned and shredded.
